The Center for Shared Decision Making is a US organization dedicated to encouraging doctors and patients to make decisions together.

Some healthcare decisions are easy. Others are not. Often, there is more than one choice, and each will have its own risks and benefits. The “right” choice depends on what is important to you. We can help.


The Center is based at the Dartmouth-Hitchcock health care provider in north-eastern US.  Due in part to the support of the Foundation for Informed Medical Decision Making, there is no charge for services.

Services for patients

  • A decision aid library of helpful DVDs and booklets on more than 30 topics
  • One-on-one decision support counseling using our Health Care Decision Guide to help you work through the decision. You can also use this worksheet on your own.

Patient decision aids are educational tools that give you information about your options and encourage you to think about what is important to you as you make your healthcare choices. These paper, DVD or web based tools may be used before, after or during your medical appointment.
